In subscribing to this program you, and all members of your household, will receive emergency ambulance services at no additional charge in our district and many other parts of Oregon. Lyons Fire-Med is not insurance but prepaid coverage in excess of any health benefit or medical benefit. Please note that some districts and private ambulance companies are not part of the Fire-Med program.
Those wishing to participate in this program must first submit an application and payment to the Lyons Rural Fire District.
Coverage is for an entire year beginning July 1st and ending June 30th. The annual open enrollment period is during the months of May & June, however, if you miss open enrollment you can still join. The fee remains the same. In the case of late enrollment, your coverage will begin the date your payment is received and will end on June 30 of that coverage year. Enrollment is limited to residents in the Lyons Ambulance service area and covers transports only by the Lyons Ambulance in its service area.
Lyons Ambulance service area includes: Lyons, Jordan, McCully Mountain, Mehama, Elkhorn, North Fork, Mill City, Gates. Please call to verify you live in the Lyons Ambulance service area.
We will continue to bill your insurance for you and will accept their payment as payment in full for services rendered. Prepayment of membership takes care of deductibles, co-pays, etc. Services must be medically necessary. Covered services include but are not limited to: emergency pre-hospital medical care, ambulance transportation, and transfers that are medically necessary and authorized by a physician to Santiam, Silverton or Salem Hospitals. To be covered, all emergency services must originate within an area of the state that participates in the Oregon Fire-Med Network. Services NOT covered include but are not limited to: transportation for non-medical situations when a private vehicle, taxi, or wheelchair and stretcher van service can be used, transport to/from doctor's offices or clinics, transport from nursing homes for treatment normally provided in the nursing home, and transport home from a medical facility when the patient's condition does not warrant an ambulance.
